Social prescribing is part of the new service model for personalised care outlined in the NHS Long Term Plan and integral to the changes in primary care and community-centered support. The expansion of the number of social prescribing link workers creates a need for training, resources and support to ensure a highly competent, sustainable and agile team.

Although in its early development stages this is an exciting addition to the PMA online portfolio.

Initially the programme will consist of 3 online modules consisting of several units. Each unit will take a maximum of 1 hour to complete and, as such, the entire programme will involve approximately 15 hours of learning time. Our aim is to provide information, support and clarity for those who are working in this evolving role and multi-agency environment.

The 3 modules are:

Module 1 Social Prescribing – theory and practice

This will explore the Personalised Care Model, Social Prescribing and how it fits in with the wider health and social care framework. The role of the Link Worker will also be examined.

Module 2 Relationship building and collaboration

Units include the referral process, how to develop creative links and networking relationships as part of multi-disciplinary teams and the implications of working with a wide-range of individuals.

Module 3 Managing Information and Data

This module covers areas such as managing caseloads and support plans, record keeping and measuring the impact of your role.

Numerous links and references to further resources will be included to provide the opportunity for further research.

The aim is to offer this as a pilot programme to a select number of existing link workers to obtain relevant and constructive feedback before we roll out nationally. We will then offer further modules for more experienced Link Workers.

  • Level  3: Level 2 English and numeracy skills recommended. Some existing knowledge of social prescribing and VCSE organisations would be advantageous
